FReferrals, Authorizations, & Payment Options

A prescription for therapy from your referring doctor is required before your first appointment. Our administrative staff will assist you in obtaining an authorization from your insurance company. Therefore, you will know how many visits will be covered as well as your financial obligations.

Our clinic accepts many PPO & HMO insurances, Medicare, Tricare as well as third party contracts for workers' compensation cases. All insurance co-payments are due at the time of your visit. We accept cash, checks, Visa, MasterCard, American Express, and Discover Card for your convenience. We will help in any way we can to assist you in handling claims. If your insurance policy has a high deductible, please let us know so we may discuss your options. We offer discounted cash rate for private pay.

California Orthopaedic Institute provides a comprehensive rehabilitation clinic conveniently located adjacent to our main facility. We have free and convenient parking, a bilingual staff, and prioritize open communication and follow-up with your referring healthcare provider.

Our physical and occupational therapy staff are committed to meeting your therapy needs with an exceptional level of professional expertise and personalized care. We strive to provide each patient with quality, individualized hands-on approach to treatment. Holly Coulson, OTR/L, CHT

Occupational Therapist and Certified Hand Specialist

Holly is the Occupational Therapist and Certified Hand Specialist on staff at California Orthopaedic Institute Physical and Occupational Therapy Clinic.

She graduated in 2002 from Colorado State University with a Bachelor of Science Degree in Occupational Therapy. In 2008, Holly earned her accreditation from the Hand Therapy Certification Commission and has since been a Certified Hand Therapist. She is a member of the International Academy of Orthopedic Medicine & the American Society of Hand Therapy.

Specializing in post op rehabilitation and custom splinting for fingers & hands, she treats her patients with an exceptional level of professional expertise and personalized care.
